Appointment No-Show Policy
As a courtesy to our patients who wish to see a doctor as quickly as possible, patients who are unable to keep their scheduled appointment must call our office one business day before the scheduled office visit. If a patient “No Shows” for an appointment, the patient could be billed for the full visit for the missed office visit. This is not a billable charge to the patient’s insurance company; it is the personal responsibility of the patient.

Emergency Care:

  • If you feel as though your condition is life threatening, contact 911 immediately for transport to the nearest hospital or emergency clinic.
  • In the event you go to the hospital emergency room, please ask them to notify your physician or clinician as soon as possible.

To ensure that your visit to our office goes as smoothly as possible, please take a few minutes to assemble all the items you will need to have on hand when you arrive. Such preparation will ensure not only that you receive the best care possible, but that any associated issues, such as insurance and billing, are handled efficiently and correctly.

Please bring the following:

  • Your health insurance card(s) and Photo Identification.
  • A list of any medicines you have been taking.
  • A list of any known food and/or drug allergies.
  • Copay and Payments are due the day of the appointment by (cash, personal checks, Visa, Discover, or Master Card are accepted.).

APPOINTMENT NO SHOW POLICY

If a patient is non-compliant with appointments, we reserve the right to dismiss that patient from the care of Synergy of Monticello Inc.

Synergy of Monticello Inc. has a calling system that calls patients 3 days before his or her appointment as a reminder call, in which the patient can confirm his or her appointment or reschedule it.

Commitment to Treatment: 
Regular and punctual attendance and compliance with the treatment plan (including medication) are essential to accomplish our goals. Frequent cancellations, appointments missed without prior notification and noncompliance with treatment recommendations are extremely disruptive and will impede your progress. These occurrences will lead to termination of your treatment. Hopefully, this will not be the case. We look forward to our meeting, and perhaps deciding to work together in the near future.
